! This position is primarily telephonic- work at home opportunity; however, PRN visits will be required when additional follow up is deemed necessary.
Work Schedule is Mon- Fri dayshift, NO weekends or holidays!!

Come join our growing Case Manager RN team in the Monroe are of Louisiana! As a case manager with OptumHealth, you'll have the opportunity to assist our members in managing their overall health by providing education, access to community based resources and referrals as needed.
Primary responsibilities of a case manager include:
Conducting initial and follow-up assessments within designated timeframes on members identified as having complex case management needs
Assessing the member's current medical and social circumstances to identify any gaps or barriers that would impact compliance with thei! r prescribed treatment plan
Engaging the member, thei! r family or caregiver as well as other healthcare providers to assure that a well-coordinated treatment plan is established
Facilitating problem resolution with members, providers, and other agencies or entities as needed
Utilizing holistic approaches to member care and integrating the member's life and motivational goals into the treatment plan
Prioritizing care needs, setting goals and developing a treatment plan (or plan of care) that also addresses gaps and/or barriers to care and uses evidence-based practice as the foundation
Tracking the member's health status and progress in achieving clinical and personal goals
Initiating PRN calls when key gaps are identified that require additional nursing follow-up
Providing education, information, direction, and support related to care goals of the member
Interacting with Medical Directors on challenging cases
Coordinating services as needed (home health, DM! E, etc)
Encouraging members to make healthy lifestyle changes
Making 'welcome home' calls to ensure that discharged member receives the necessary services and resources

Requirements
Requirements include:
- Must reside in Louisiana
- Current, unrestricted RN license in the State of Lousiana
- 2+ years experience in a hospital setting, acute care, direct care experience or experience as a telephonic Case Manager for an insurance company
- Must have computer skills to include proficiency in MS Office and the Internet. explorer and email system. !

÷ Sort and use Auto Filters in Excel Spreadsheets
÷ Hide and Unhide Columns in Excel Spreadsheets
÷ Use of Microsoft Outlook: Manage Email and establish folders, Attach Documents in Email to send and open Attached Documents in Email, Manage Calendars and set appointments
- Typing skills: Must be able to talk and type at a rate of 25 wpm
- Internet Connection: Must have the ability for UHG to install high speed Internet service at location of home office. Selected candidate needs to meet UHG telecommuter agreement policy standar
- Bachelor's Degree
- A background in managed care
- Case Management experience
- Certified Case Manager (CCM)
- Experience/exposure with discharge planning
- Experience in utilizatio! n review, concurrent review and/or risk management
- Strong orga! nizational skills and multitasking abilities will be keys to success
