We are currently looking to add a dynamic Clinical Field Staff Specialists to our team.

The Clinical Field Staff Specialist is responsible for the receipt/input/review/completion of all referrals. Contacts physicians/referral sources regarding patient care and non-admits. Does initial assignment of evaluation visits. Contacts patient/family to notify of services that will be provided. Prepares patient list for and participates in weekly Case Conference. Assists Billing Specialist with timely completion of billing tasks.

The Clinical Field Staff Specialist reports directly to the Director of Operations.

Essential Duties and Responsibilities:

  • Utilizing clinical operational software inputs/reviews/ completes ! all patient referrals. Reviews all electronic documentation to! ensure coordinating the total care plan with the appropriate resources, conducting periodic re-assessments of client needs, revising the plan of care and monitoring employee performance.
  • Provides ongoing assessment in case management of clients, assuring an accurate evaluation of patient needs.
  • Facilitates and follows up on Face 2 Face Encounters ensuring compliance and completion.
  • Does initial assignment of clinical personnel for evaluation visit. Ensures that all orders that address scheduling and frequency are appropriately modified on the calendars. Maintains optimum productivity levels; monitors daily and weekly schedules matching needs, abilities, and territories to the assignment.
  • Maintains ongoing communication regarding patient care with staff physicians, referral sources, caregivers, patients, facility administrator and staff to facilitate coordination of care.
  • Assists Centralized Insurance Specialist with verification! of benefits and obtains authorizations on all insurances and Medicaid as needed.
  • Assists Billing Specialist with the timely completion of assigned billing tasks.
  • Assures compliance with Federal, State, and local statutes regarding competency evaluations and supervisory visits.
  • Maintains current and accurate records in accordance with policies and procedures, Federal, State and local guidelines.
  • Under the direction of the Director of Operations assists with recruiting, hiring, and directing the training of field staff.
  • Participates in the planning and presentation of in-service education programs, keeping the staff current with changes in health care procedures and company policies.
  • Prepares patient list and participates in Case Conferences with all disciplines. Notifies Patient Services Coordinator of all patients that are to be recertified or discharged. Enters electronic coordination notes into all patient records wit! hin one day of completion of Case Conference.
  • Serves as a role! model and clinical teacher for field staff.
  • Protects the integrity of the organization, patients and co-workers by maintaining confidentiality of all patient records and business information.
  • Maintains a safe work environment to minimize risk to patients and staff by complying with all Safety/Risk/OSHA policies and procedures.
  • Maintains and contributes to the efficiency of operations by consistently complying with all company and regulatory policies, procedures and guidelines.
Requirements รข€¢ Have current license as a Registered Nurse in the practicing state.

  • Minimum of 2 years Medicare home care experience, including 1 year of nursing supervisor or administrative experience.
  • Expert working knowledge of Medicare covered home health care and other insurance programs, regulations and guidelines regarding home health care.
  • Knowledge of Federal and State regulations.
  • Maintain current health req! uirements.
