NHS Human Services, through its subsidiaries is one of

community-based human services. With nationally
recognized programs in multiple states, NHS offers a
full range of integrated services to children and adults
in the areas of mental health, addictive diseases,
autism, intellectual and developmental disabilities,
juvenile justice, treatment foster care, education and
other specialized services.
The Team Leader establishes administers, and directs the
program of the Assertive Community Treatment Team, a
self contained clinical team which assumes
responsibility for directly providing needed treatment,
rehabilitation, and support services to identified
clients with severe and persistent mental health
illnesses: supervises and evaluates the
multidisciplinary team in conjunction with appropriate
psychiatric support to ensure servic! e excellence and
courteous, helpful and respectful service! s to program
clients; and functions as a practicing clinician on the
team.

  • Direct the day to day clinical operations of the ACT
Team including coordinating staff work schedules to
ensure proper coverage is met
  • Facilitates Daily Team Meeting and maintains daily
log of meetings.
  • Maintains hospital and incarceration log
  • Direct and coordinate the client admission process
and treatment, rehabilitation, and support services
in coordination with the psychiatrist. Scheduling
admission interviews, developing and coordinating
initial assessments and treatment plans
  • Direct and coordinate assessments of the following:
Family Structure and relationships, Substance use
assessment, Education and Employment, Social
development and functioning, daily living
activities, nursing assessment, and Psychiatric
assessment
  • Provide clinical sup! ervision of ACT team members as
well as general oversight and supervision of the
assessment and treatment plan for each client This
includes weekly individual supervision of team
members and at least monthly, this should occur in
the field
  • Initiating and maintaining relationships with staff,
clients, community resources, and other outside
human service agencies as needed
  • Ensure client medical records are in compliance with
agency Medicaid/Medicare, and any outside governing
agency policy and standards as well as ensuring that
all third party payment requirements have been met
  • Assist with staff recruitment, interviewing, hiring,
performance evaluations, CQI teams, Global
Monitoring process, enforcing company policies,
staff training, and other employee relation issues
as needed
  • Participate in cross training with other specialists
! as deemed necessary by supervisor
Rate of pay is based on experienc! e = $52k +
****$$ Travel stipend paid in addition to salary $$****

Qualifications:
  • Master’s Degree in Social Work, Counseling,
Psychiatric Rehab, Nursing, or Psychology required.
  • This candidate must have strong clinical skills.
  • Minimum 2 years post Master's experience working
with the adult severely mentally ill population
required.
  • Two years supervisory experience strongly preferred!
  • Good organizational skills are a must.
  • License is required â€" LCSW or LPC.
Hours of work are primarily day shift - will include
shared rotation by team members of late shift and
weekend hours, plus on-call duties - split by team
members. Requires valid driver's license and reliable
transportation.
