Employed for the purpose of enhancing the nature and quality of care for ! patients. . Major areas of responsibility include: assessing and screening patients, utilizing specific tools; monitoring compliance with plan of care; encouraging lifestyle and
behavior modification; providing primary and secondary education for the patient and patient's family; collaborating with staff and physicians to ensure continuity of care in the outpatient selling; and managing the day to day operations. Demonstrates actions consistent with Ochsner Health System's Expectations" as duties are performed on a daily basis.

Ill. MINIMUM QUALIFICATIONS:A. Education, Experience & Training:
Graduate of an accredited school of nursing wilh current Louisiana licensure.
Current licensure or permit In State of Practice.
BSN with 3 years experience or RN with 5 years experience.

CPR Healthcare Provider Certification.
1 year supervisory or leadership experience required.
