The Licensed Practical Nurse effectively maintains the Standards of Practice and Care, utilizes the nursing process using direct and indirect care and promotes collaboration with physicians and other disciplines while under the supervision of a Registered Nurse. Major Duties: Meets organizational expectations as defined by the Employee Standards of Performance, hospital and department policies and procedures, and the hospitalâs strategic goals. Verbalizes a clear unde! rstanding of continuous quality improvement and customer satisfaction measures and actively participates in improvement initiatives. In collaboration with RN assesses/reassess patientâs condition based upon observation of patient and interview of patient and/or family, physicianâs diagnosis and data, diagnostic results, and relevant data from other healthcare providers.
Delegates responsibility to appropriate personnel, as well as, accepts delegated responsibility from shift coordinator. Documents all aspects of patient care and maintains patient records. Provides patients and family members with relevant health care education and instructions regarding treatments and procedures including pre/post hospital care as appropriate. Maintains a clean and organized work environment.
Assists in the orientation and training of new and tenured personnel, reassigned nurses, student nurses, patient care assistants, and faculty; serves as a preceptor when requested! . Completes all mandatory and elective competencies and contin! uing education as required. Attends unit meetings as scheduled. Responds to internal / external disaster as directed.
Accountable for nursing practice to the Chief Nursing Officer or designee in the facility in which practice takes place. Evaluates and updates the multidisciplinary care plan daily. Communicates clearly and effectively with all multidisciplinary team members. Participates in staff meetings, education programs, committees, QI activities and mandatory in-services.
Completes FIM scores daily. Unit specific skills checklist and competencies completed prior to end of 90 day probation evaluation. Performs other duties as assigned or directed to ensure smooth operation of the organization. Education/Knowledge/Skills/Abilities: Preferred: 1 year Medical/Surgical experience.
1 year In-patient Rehab experience. Competence in FIM scoring. Required: Current knowledge of the nursing role and scope of practice as defined by the State of Louisian! a Nurse Practice Act and the Louisiana Board of Nursing. Current knowledge of American Nurse Association (ANA) code of ethics.
Knowledge of current trends and demonstration of standards of care in assigned unit. Successful completion of mandatory requirements including unit proficiencies/competencies. Demonstration of effective problem solving, decision-making interpersonal and teamwork skills. Demonstration of effective verbal and written communication.
Graduate of an accredited LPN program. Demonstration of positive customer service.
