The Student Health Nurse is a LPN who will be active in working under the supervision of the School Nurse. The LPN will assist students with special needs to ensure they participa! te to their fullest potential in the classroom. The LPN will provide high quality nursing care and support the lead teacher within the classroom while communicating in detail with parents and the School Nurse to ensure the best continuum of care for a disabled student.
Position duties will include in classroom delivery of daily health services including ADL’s. The incumbent will utilize the Nursing Process to deliver quality care consistently and effectively. S/he will coordinate care in the classroom, communicate with the School Nurse and the multidisciplinary team to establish and maintain a safe environment for the student conducive to learning. Duties will be performed in accordance with standards of professional licensed practical nursing care, scope of practice, district/state board of education policies and procedures and Louisiana state law regarding Licensed Practical Nursing with deference to the state Nurse Practice Act.
Position Responsibi! lities:
Utilize the nursing process components of asse! ssment, diagnosis, outcomes identification, planning, implementation, and evaluation to care for ill children during the school day.
Implement procedures on campus in conjunction with school/district policy
Follow individual health care plan , 504 or Medical orders
Conduct annual screenings
Conduct follow-up screenings for vision or hearing screening failures
Meet as a member of IEP, 504 and health and wellness teams, gather information and give input for intervention strategies
Maintain a medication administration folder with valid physician orders for specialized care of students including, but not limited to special medical procedures during school hours (e.g., gastrostomy feeding, tracheostomy care, blood glucose monitoring, and catheterization) as well as administration of medication
Physical Requirements:
Must be able to stand, walk for extended periods of time
Must be able lift up to 3! 0 lbs during the course of duties
Must be able to climb stairs throughout the day in the course of duties
Qualifications:
Licensed by the state of Louisiana as a Practical Nurse
Valid, unencumbered Louisiana LPN license
Experience in school setting or pediatrics preferred
