GHG is looking for a Family Nurse Practitioner or a Physician Assistant to provide services at the Shreve! port Job Corps Center, Louisiana.
Available Positions : Nurse Practitioner (NP) or Physician Assistant (PA).
Place of Performance: Shreveport Job Corps Center, Shreveport, Louisiana 71109.
Work Schedule: Part-time, 6 hours per week. Work will be performed during regular business hours, Monday through Friday, at times and days mutually agreed upon by the Dentist and the Center Director or designee. Services are required 48 weeks per year, except during the two (2) week annual Christmas Break and July 4th periods. The NP must provide her proposed schedule (days).
Period of Performance: Beginning July 1, 2013 to August 31, 2014 and it may be extednded upon acceptable performance and all necessary state reviews and approvals.
Description of Services (includes but it is not limited to):
The development, implementation, and monitoring of the total center health and wellness program, to help studen! ts become and stay employed.
Supervision - in additio! n to the specific coverage outlined, the contractor would be responsible for the medical supervision of all health care services personnel, including the examinations and treatment rendered to the students. The Physician and NP OR PA is responsible for monitoring chronic illnesses.
Family Planning Counseling - the contractor, in cooperation with an appropriate specialty consultant, would be responsible for developing and supervising ongoing education for health services personnel, providing counseling services, and personally rendering direct patient counseling in this area to the students. State and private contracting agencies may be utilized in this area.
Continuing Medical Education - the Physician or NP/PA would be responsible for the development and supervision of continuing medical education (on the job) for Job Corps center health services personnel.
Health Care Guidelines - the Physician or NP/PA would be responsible for periodic review, ! update and approval of health care guidelines and other protocols followed by the health and non-health personnel in treating students.
Quality Assurance Monitoring - the Physician or NP would be responsible for ongoing review of medical care and medical record keeping as practiced by the health and non-health services personnel.
Hazard Evaluation - the Physician would be available to evaluate and make recommendations regarding physical, chemical, biological, etc. hazards and other areas of occupational medicine such as safe job practices and accident prevention with regard to the vocational training program carried out by the Job Corps center.
The NP/PA Will Also Provide Direct medical services to students:
1. Assessment and possible diagnosis of illness and injury in an ambulatory facility. Cursory medical inspection for each student within the first 48 hours after arrival on center. A definitive medical examination and health history o! f each student, within 14 days of arrival on center. Routine laboratory! work that can be done on center, e.g., hemoglobin or hematocrit, urine sugar and albumin, and pregnancy Immunizations as indicated Tuberculin skin test (Mantoux) Vision and hearing screening Daily walk-in clinic and appointment system for above and for episodic illness or injury assessment by contractor and/or nurse Inpatient unit (during office hours) for minor conditions, such as upper respiratory infections or flu symptoms
2. Treatment, as highlighted below, will be provided when necessary. Third party payor information will be given to providers when off center care is required. Primary emergency care for illness and injury, including first aid and CPR, and secondary care within capabilities, e.g., injection of epinephrine, and immediate transfer to hospital emergency room for specialized diagnosis and treatment, if needed Treatment of urgent and other conditions not needing specialized care, within capabilities of contractor and nurse, such as contusions and ! abrasions Referral to off-contractors for detailed specialized assessment regarding on-center management of asthma, uncomplicated diabetes, normal pregnancy, etc. When students need prescriptions not provided in the basic formulary, local pharmacies will be given insurance information whenever possible If a student sustains an on-the-job injury that require
Position Requirements/Qualifications
License/Certifications: Shall have and maintain an active, valid, unrestricted NP or PA State license (with no limitations, stipulations or pending adverse actions). Candidate must registered with the State Examining Board or certified by the National Commission on Certification of Professional Assistants Valid DEA license. Must have proof of Current Basic Life Support (BLS).
Education: shall have a graduate from an accredited NP or PA program.
Citizenship Requirement: Must be a citizen of the United States or have a Green Card.!
Experience: Prior work experience in dealing with adol! escents and young adults is required. A statement of Experience dealing with adolescents is also required. Provision of health services to young adults who are socially and economically disadvantaged is desired. Previous practice in settings such as schools, colleges, institutions, military or correctional institutions is desired. Admission and attending privileges at local hospitals or specialty clinics is desired. Academic affiliation with nearby professional schools or colleges is desired.
Immunizations: Submit a copy of a current physical examination (within the past 2 years) with current immunizations for MMR and DT (measles, mumps, rubella, diphtheria and tetanus.
Background Check: Background check and fingerprints will be conducted in order to obtain security clearing.
